Position Title: Certified Substance Use Disorder Case Manager

Report To: Program Director

Salary: Up to $28/hr. depending on experience, Crossroads pays 100% of health insurance cost, paid vacation and sick time

Hours: 40 Hours, M/T/F 1-9, OFF Wednesday and Thursday, Sat-Sun 8-4 Must be able to work holidays as well.

Must have:

Roles & responsibilities

Case management of 6 – 10 clients as they transition from incarceration into the community

2. Work one-on-one with clients, conduct group activities, and provide weekly support

3. Provide direct clients with services, including development of individual treatment plans, weekly goal setting, progress updates, advocacy, information and referrals

4. Maintain accurate records, including documentation of all services provided, monthly program reports on activities, upkeep of client files, and tracking information

5. Attend all meetings as required and all supervisor requested trainings

6. Assist with the development of a warm and positive environment that promotes resident well- being

7. Communicate professionally and appropriately with staff and the public by observing confidentiality, staying focused on job tasks and demonstrating active listening skills.

8. Demonstrated ability to be an effective team member by communicating openly, cooperating with other staff, demonstrating positive attitudes and working together to solve problems

9. Demonstrated ability to work effectively under supervision by being open to feedback, reporting on activities as directed, staying within the job description and following through on suggestions

Qualifications

Certified AOD RADT (with at least 1,000 hours) or CATC/CADC from an accredited provider

High school diploma or GED

Two years education in behavioral or social sciences or related experience

Ability to relate in a non-judgmental manner to women on parole

Knowledge of criminal justice system and substance use disorder

Facilitation Skills

Excellent communicator

Positive Attitude

Written Skills

Must be over 21 and possess a valid CA driver’s license

3 References must be provided

PLEASE NOTE: Crossroads, Inc. and related programs require a Live Scan background check and a TB clearance prior to start date. Any job offer is not finalized until the candidate passes the background check. All new hires must also get a physician to provide a sign off on a health screen. Prior to start date Case Manager must take Motivational Interviewing, CPR and Standard First Aid, HIPPA, and Sexual Harassment These can all be done online.
